Please complete this form for any students you believe would benefit from a meeting with the success coordinator(s). Please include the student’s ID number in your report and provide as much detail as possible. Sometimes we know that you just want to put a particular student on our radar, and you can use this form for that as well.
This form replaces the SSI form in situations where there is an academic only issue. If the student is at risk of failing the course you would submit both an SSI and a MIGSON Referral; in situations where there is an academic concern AND other issues you should submit the SSI addressing those other issues and note that you've submitted a MIGSON Success referral for academics. To recap you would only complete an SSI if there is a major concern about the student's ability to persist.
Examples could include: they are not utilizing suggested support services and programs, a suspected disability but no letter of accommodations, student is at risk of failing the course (this would require an SSI and a MIGSON Referral), student has disclosed housing/food insecurity, student discloses death or hospitalization of immediate family member. No more SSIs for each failing exam (those would go to MIGSON Referral) but keep them for everything else pretty much.
This MIGSON Referral form also does not replace meeting with your student as course faculty or academic advisors. We request if you are referring a student in your course to meet with us that you meet with them first and update us regarding what you learned and have already covered for the student. If you've submitted a referral stating you will meet with the student soon, please update us as soon as you've met with the student so the Success Team can update their records.
Completing this form does not take the place of an SSI. If a SSI is needed, you must fill out a separate form for the University.
